Insurance and Claim Forms

We participate with most insurance plans and will submit claim forms on your behalf. For your protection, and to assure accurate billing, please bring your insurance card and identification with you to each visit. Please notify us promptly of any changes in your name, insurance coverage or contact information.

Our staff will discuss co-pays and other payment responsibilities with you.

Co-pays and payments for office visits are expected at the time of service. We accept Visa, MasterCard and Discover credit cards.

If you have a managed care plan, referrals are required to see specialists and for certain diagnostic test and procedures.

Please feel free to discuss any insurance questions with our staff.

Hospital Consultations

Our practice is proud to be affiliated with Abington Memorial Hospital (AMH) and its highly qualified medical staff.  We have great confidence in the specialists, ancillary services and inpatient care at AMH. We also provide consultation services at other hospitals. Should patients need hospitalization, our physicians will be involved in the coordination of your care and will work with attending physicians, other specialists and clinicians. As a teaching facility, AMH is committed to training the next generation of physicians and residents may be involved in your care.

When you are discharged, you will receive detailed discharge instructions and your physician will get a report summarizing the care you received. To ensure appropriate follow-up, we encourage you to contact our office to schedule an appointment.

Neuropsychologic Services

Neuropsychologic testing is available for neurocognitive disorders, including traumatic brain injury, cerebral vascular disease, multiple sclerosis, dementia and other neurological conditions. These services are provided by a doctorate-level neuro-psychologist.

To schedule an appointment, please call 610-279-7443 for the Norristown office and 215-836-3462 for the Flourtown office.
