Indicate HF - A prospective, randomized, multicenter clinical trial. SJ4 Post Market Approval Study - Purpose is to characterize the chronic performance of the (SJM) SJ4 connector and RV high voltage SJ4 leads in patients that have an approved indication, as per ACC/AHA/HRS guidelines, for implantation of an ICD or CRT-D system for treatment of heart failure or life threatening ventricular tachyarrhythmia, and who are not currently participating in a clinical investigation.
