Discharge
A physician will order your discharge, once you are ready to return home. In preparation for discharge, the staff will work with you to prepare for your return home. The discharge process takes some time as you will receive instructions about follow-up care, such as diet, medications, level of activity, and follow-up appointments with your physician. A nurse and physician will review these instructions with you. Please be sure to ask them about any concerns you may have. If you have any special needs such as home health care or home medical equipment, the Case Management office will assist you with these needs during the discharge process.
