Case Management

Case Management is collaborative process of nurse case managers, licensed social workers, physicians and other members of the healthcare team working together to coordinate the transition from acute hospital care to either facility or home based care.  The goals of coordination are to ensure both optimal patient and hospital outcomes.

The Case Managment Department is available to help with the problems and stress that often accompany illness or hospitalization. Professional staff, including licensed social workers and registered nurses, provide support and guidance in dealing with an illness or treatment and assist with making decisions about discharge plans. This professional staff is experienced in working with extended care, assisted living, residential, skilled nursing facilities, durable medical equipment providers and community and home heatlh agencies.  The nurse case manager reviews the tests and procedures ordered during the hospital stay ensuring the appropriate utilization of resources, quality of care prescribed and reimbursement of the services provided based upon specific patient diagnosis and treatment plans.  The case managment team acts as the patient and family advocate along the continuum of care.
