Preadmission Registration

Thank you for selecting Abington Memorial Hospital for your upcoming admission. If there is anything we can do to make your stay more pleasant, please tell a member of our staff.

To make your admission as easy as possible, we request that you take time to read this page and follow the instructions so everything will be ready when you arrive. If you have any questions, please feel free to contact us at admissions@amh.org.

Pre-registration

When your physician schedules a reservation for admission or short procedure stay, you may use this web page as a starting point to pre-register for your visit. Our goal is to make the admission process easier for you by receiving and processing your registration information prior to your visit.

You may start this process by downloading and printing the preadmission registration form (pdf), filling it out, and mailing or faxing the completed form back to us at 215-481-8185. When we receive your information we will process your pre-registration and call you to set up an appointment with the Preadmission nurse.

Maternity Admissions

To pre-register, your doctor must be on staff at this facility.
Prior to your delivery you will need to obtain and complete various pre-registration forms. Begin by downloading and printing the preadmission registration form (pdf), filling it out, and mailing or faxing the completed form back to us at 215-481-4439.

When we receive your information we will provide you with the additional forms that are needed for your and your new baby's stay. Either mail via U.S. Postal Service or bring these completed forms to us prior to your delivery.

Don't forget: Make sure you obtain pre-authorization from your health care insurance provider. Contact should be made within the first two trimesters of your pregnancy to avoid possible penalties. If you receive a letter of confirmation from your insurance company, please submit it with your preadmission forms.

Accommodations

On your admission day, every effort is made to provide accommodations in accordance with your personal preference. However, due to volume this is not always possible. If your desired accommodations become available, a transfer may be arranged. In order to provide optimal room and bed placement, please inform us of any disabilities or special needs required for your hospital stay.
