One-Call Transfer Process
Transferring patients should be a simple, speedy and safe process. That’s especially important for patients with emergent neurological and neurovascular conditions.
With just one call to the Abington Memorial Hospital (AMH) One-Call Transfer center at 215-481-8181, you can transfer your patients safely, quickly and easily.
One-Call is staffed by registered nurses and available 24 hours a day, seven days a week.
Follow this simplified process to transfer a patient:
Step 1: Connect with One-Call at 215-481-8181.
Step 2: Provide clinical information for the patient:
- Vital signs
- GCS (Glasgow Coma Scale)
- Test results
Step 3: Physician-to-physician communication
Step 4: AMH arranges transport (alert if any special transportation needs)
Step 5: Prepare patient for transport and include:
- Transfer consent form
- Patient’s medical history
- Ancillary testing results
- Diagnostic studies
